     • We know you're not a new patient at our clinic, but in the medical world, a gap of 3 years means we we need to do the intake again.​

  2. HIPAA Consent

  • and one (or both) of the following if your appointment relates to an open claim:

    • Motor Vehicle Accident Questionnaire.

    • ​Workplace Injury Questionnaire

• Plan on showing up 10 minutes prior to the appointment time. We’ll need to make a copy of your drivers license and insurance card; take your height, weight, blood pressure, and temperature; and get everything in order before your designated appointment time.
