    2. HIPAA Consent

    • and one (or both) of the following if your appointment relates to an open claim:

      • Motor Vehicle Accident Questionnaire.

      • ​Workplace Injury Questionnaire

  • Please arrive 10 minutes early. We’ll need to copy your driver’s license and insurance card, check your height, weight, blood pressure, and temperature, and make sure everything is ready before your appointment.
