• Schedule an “Exam: Current Patient” with Dr. Nelson or Dr. Johnson from the drop down menu at the top of the page.

• Plan on showing up 5 minutes prior to your appointment time.

• If your injury isn't part of an auto or workers' compensation claim, you don't need to fill out any paperwork before your visit. However, if this appointment is related to an open auto or workers' compensation claim, fill out the:

  1. New Patient Intake Form

     • We know you're not a new patient, but your insurance company will want all of this information.

  2. and one (or both, as applicable) of the following:

     • Motor Vehicle Accident Questionnaire.

     • ​Workplace Injury Questionnaire​​
